OAM: Update LibreNMS switch device hostnames (DNS rename)

Description
Switch DNS names were renamed but LibreNMS still uses old hostnames that no longer resolve, causing SNMP polling failures for 4 devices since April 2026.
Old -> New mapping:
- pfv-core-sw01.knel.net (dev 1) -> pfv-r5-core-01.knel.net
- pfv-r3-mgmt.knel.net (dev 6) -> pfv-r3-tor-mgmt-01.knel.net
- pfv-r3-stor.knel.net (dev 7) -> pfv-r3-tor-stor-01.knel.net
- subodev-torsw01.knel.net (dev 8) -> verify (may be unchanged)
Fix: update hostname field in LibreNMS devices table (preserves device_id and all historical data). Verify SNMP polling restored.

Completed LibreNMS switch device rename. Updated hostname field in devices table (device_id preserved, all historical data retained):
dev 1: pfv-core-sw01.knel.net -> pfv-r5-core-01.knel.net (Dell PowerConnect 5448)
dev 6: pfv-r3-mgmt.knel.net -> pfv-r3-tor-mgmt-01.knel.net (Neyland 24T)
dev 7: pfv-r3-stor.knel.net -> pfv-r3-tor-stor-01.knel.net (Neyland 24T)
dev 8: subodev-torsw01.knel.net -> pfv-r6-mgmt-01.knel.net (Neyland 24T)
Root cause of polling failure: old DNS names no longer resolved after switch rename, causing SNMP to fail (not a cron issue). SNMP community kn3lmgmt confirmed working to all 4 switches via new DNS names.
Verification: all 8 devices now polling successfully. last_polled timestamps current as of 2026-08-11 12:53. Poller cron cycle will maintain going forward.
Note: subodev-torsw01.knel.net no longer exists in DNS — the rack 6 mgmt switch is now pfv-r6-mgmt-01.knel.net (DHCP reservation 192.168.0.8, MAC 00:14:22:69:18:a7).
